Stage 2: Registration & Grant of License (Form No. 2)

Once the layout plan is formally approved by the department, you must submit Form No. 2 (Notice of Occupation) along with these supporting documents:

1. Legal Entity & Representation Proofs

• Constitutional Credentials:
• For Corporate Entities: Certificate of Incorporation, Memorandum of Association (MOA), and Articles of Association (AOA).

• Power Allocation Proof: A copy of the formal power sanction load order from TANGEDCO and the latest electricity bill specifying the maximum connected Horse Power (HP) capacity.

3. Safety, Environmental, & Structural Certifications

• Form No. 5 (Certificate of Stability): A mandatory structural soundness certificate issued in the exact state-prescribed format, signed by an empanelled DISH-Competent Person or a licensed Structural Engineer.
• TNPCB Clearances: Valid Consent to Establish (CTE) and Consent to Operate (CTO) issued by the Tamil Nadu Pollution Control Board.
• Fire Safety NOC: A No Objection Certificate issued by the Tamil Nadu Fire and Rescue Services Department.
• Specialized Safety Disclosures (If handling Hazardous Processes under Section 2(cb)):
• A comprehensive On-Site Emergency Plan.
• A formal Health & Safety Policy (Mandatory for units employing 50 or more workers).
• Approval from the Petroleum and Explosives Safety Organization (PESO), if applicable.

1. Register on DISH Portal — Create an account on https://dish.tn.gov.in/.
