- Form IX, with the nomination of
persons by the company authorized to be responsible for food safety.
- Self-declaration form confirming compliance
with FSSAI regulations and product standards.
- Proof of annual turnover to determine whether
the Basic, State, or Central license applies.
- IEC or Import
Export Code License, if the business imports or exports food
products.
- Employee list with medical
fitness certificates for those handling food directly.
- Declaration Form B, duly signed by the
proprietor, partner, or authorized signatory.
- Certificate of hygiene and
