Documents Required for FSSAI License in Telangana

• Documents for Basic Registration or License:

1.   Photo id proof of the FBOs;

2.   Possession of proof of business premises, such as NOC, Rental Agreement from the owner of rented premises, latest utility bills, etc.;

3.   List of food items processed or manufactured;

4.   Details of Bank Account;

5.   Business Certificates like Incorporation Certificate, Partnership Deed, Shop & Establishment License or other Business Registration Certificate;

6.   Food safety management system plan;

7.   Supporting documents (if required) like No Objection Certificate (NOC) by Panchayat, Health NOC, License copy from the manufacturer, etc.

• Documents for FSSAI State License:

1.   Form B duly signed and completed;

2.   List of Partners, Directors, or Proprietors with address, contact details, and photo id;

3.   Analysis of work report to be used in the process to confirm the portability;

4.   Plant of processing unit showing the dimensions & operation-wise are collection;

5.   Authority letter from manufacturer appointed a responsible person name & address;

6.   Copy of certificate attained under Coop Act 1861 or Multi-State Coop Act, 2002;

7.   List & name of machinery and equipment used with the number & installed capacity.

• Documents for FSSAI Central License:

1.   Form B duly signed and completed;

2.   List of Partners, Directors, or Proprietors with address, contact details, and photo id;

3.   Analysis of work report to be used in the process to confirm the portability;

4.   Plant of processing unit showing the dimensions & operation-wise are collection;

5.   Authority letter from manufacturer designated a responsible person name & address;

6.   List & name of machinery and equipment used with the number & installed capacity;

7.   Form IX;

8.   Ministry of Commerce Certificate form 100% EOU;

9.   Raw material source for meat, milk, etc.;

10.                PA or NOC document issued by FSSAI;

11.                Supporting documents for turnover & transportation proof;

12.                Declaration form;

13.                IEC document issued by DGFT;

14.                Recall plan wherever applicable.

Procedure for Obtaining FSSAI License in Telangana

An applicant looking for FSSAI License in Telangana needs to file an online form in the official portal called FOSCOS. Once you log in to this portal, open the new application option enlisted on the right side of the home page. Selecting this option will unlock the illustrative e-form on your screen, where you need to provide the following details:

1.   Annual Turnover;

2.   State where the business is situated;

3.   Business nature, i.e., manufacturer, retailer, marketers, etc.

In the next window, you have to upload all the vital documents regarding Business Registration, details of partner, premises, PAN and so on. As soon as you upload the documents, a payment gateway window will be seen on the screen, prompting you to make the payment of the application fee.
